If you are consumer, individual health care provider, hospital, or health system and you are looking at ways to bridge the gap between medical care and a persons’ social support systems, you have come to the RIGHT place.
OWN is a division of Oregon Association of Area Agencies on Aging and Disabilities (O4AD) and serves as a network hub for the 16 Area Agencies on Aging (AAAs) in Oregon. OWN was developed to help the individual AAAs create a value proposition for the social services they provide.
